Step 1: September (or when a new child joins your class)
If a teacher has a concern for a child they will need to be put in a target group with focussed support for ½ term. Additionally the TA will also work on identified GAPs during the afternoon sessions. The timetable needs to be agreed with the teacher and TA to support specific children within your class.
Step 2: Outside support
If the Rising Stars tests show the child is significantly under achieving and the class teacher wants external support (eg SENAAT), complete SEND concern form and return to SENCo. You will have a copy of this in your SEND file. SENCO will analyse the concern form and will provide further information as required.
Teacher begins the Individual Cognition and Learning Plan (IEP) based on existing GAPs and needs.
Step 3: Individual Cognition and Learning Plan
Using external report and gaps analysis, adapt Individual Cognition and Learning Plan.
*A copy needs to be stored in the SEND file, 1 sent home to parents and 1 for the TA. When the files are collected in we will make copies as required.*
Step 4: Additional Support Plan
If, after two full terms of an Individual Cognition and Learning Plan the child is still under achieving on the Rising Stars tests, the SENCo will meet with parents and the class teacher to begin writing an Additional Support Plan.
Step 5: Educational Health Care Plan
If, after two full terms of an Additional Support Plan child is still under achieving on Rising Stars tests, SENCo to request a full assessment of the child.
